Healthcare facilities, employers, businesses, and organizations that coordinate Health Vax clinics are responsible for providing accurate information and supporting the clinic process as agreed upon with Health Vax.

• Coordinating clinic dates, locations, and logistics

• Communicating clinic details to eligible participants

• Supporting completion of consent forms

• Providing accurate participant information

• Ensuring appropriate authorization for residents, patients, employees, minors, or other participants

• Providing safe and suitable space for clinic services

• Complying with applicable workplace, healthcare, privacy, resident, employee, and consent requirements

• Cooperating with documentation, billing, and follow-up workflows

Health Vax is not responsible for inaccurate, incomplete, delayed, or unauthorized information provided by a facility, employer, organization, participant, representative, or third-party platform.

Consent Forms and Participant Information

Health Vax may use digital consent forms or third-party form platforms to collect information needed for vaccine clinic services. By submitting information through a consent form or related workflow, you represent that the information provided is accurate and that you are authorized to provide it.

For minors, residents, patients, employees, or other individuals who require assistance or legal authorization, information and consent must be provided by a parent, guardian, legal representative, authorized healthcare facility, employer, or other authorized party as applicable.
